What is the Wellness Journal?
The Wellness Journal is a 28 day method for organizing your health information to aid in diagnoses, help assess the effectiveness of treatments, track progression and identify what makes symptoms better or worse.
It contains 4 weeks of journal entries and includes pages for weekly summaries and a place to make notes to share with your healthcare practitioner.
Developed by patients with chronic conditions to help you better understand the connection between what you do and how you feel.
The more connections you can find, the greater opportunity you have to make choices that improve your health.
Who should use the Wellness Journal?
The Wellness Journal is designed specifically for people who have, or suspect they have, a chronic health condition such as Rheumatoid Arthritis, Cancer, Asthma, Celiac Disease, Crohn’s Disease, Lupus, Multiple Sclerosis, Parkinson’s, Alzheimers, Chronic Fatigue Syndrome (or ME), Postural Orthostatic Tachycardia, Mastocytosis, Mast Cell Activation, Autism, Plaque Psoriasis, IBS. and anyone or who suspects they may have food or environmental allergies.
